Adult Registration & Consent Form

I am willing to take part in Arts Centre activities from the period 1 September 2021 – 31 August 2022, and for the tutor in charge or Head of Service to sign on my behalf any forms of consent required by the hospital authorities in the event of myself being ill or injured during the course of a journey or stay to the extent that a surgical operation or serum injection becomes necessary, provided the delay required to obtain a signature from emergency contact might be considered likely in the opinion of the doctor or surgeon concerned to endanger my health or safety.

All members are covered by the Salford Community Leisure Ltd third party liability insurance in respect of any claim arising from an accident caused by a defect in the premises or equipment or attributable to negligence by Salford Community Leisure or one of their employees. These arrangements do not provide personal accident cover.
